Behavioral / ADHD Testing
In our Behavioral treatments the doctor works with you, your child and your child’s teacher to help your child thrive.
Your initial visit:
The doctor will spend some time speaking with you as a family, then separately while your child completes certain activities. During this first meeting, the doctor will obtain as much information as possible about your child and your family. Afterward, she will review all the information collected to reach her diagnosis and establish an action plan.

Before your first behavioral appointment, please complete the forms below. In addition to the forms below, you will need to complete an additional questionnaire that will be e-mailed to you. Please send us a message to request it. Please note that you will need to have your child’s teacher complete a portion of the forms, so kindly plan accordingly.
