Physicals and clearance forms
We are frequently asked to provide medical evaluations and sign forms required by school districts, organizations or clubs to clear a child for certain physical activities. We are happy to complete these forms during your child's annual visit.
If your child has had an annual exam within the last year and needs a sports physical later, we may need to update their medical history and do a limited exam before providing clearance. We charge $20 for this service, payable at the visit. Please note that many insurance companies do not cover it.
